The Light House registration packet includes the following documents:
FAMILY APPLICATION FOR ENROLLMENT
One application must be filled out for each family and returned with the family fee of $100. Please make check payable to The Light House.
PARENT/STUDENT RESPONSIBILITY AGREEMENT
Each family must complete and return the agreement of responsibilities for both parents and students.
STUDENT REGISTRATION
One class registration form must be filled out for each child and returned with the $55 registration fee for each child.
FAITH STATEMENT
Each family must agree to and sign the statement of belief.
MEDICAL STATEMENT
Please fill out and return to TLH.
FINANCIAL AND VOLUNTEER AGREEMENT
Each family must read, sign and return completed sheet with other registration papers. Application is not complete without agreement with these policies.
PASTORAL REFERRAL LETTER
Please give to your home church pastor to complete and return to school either by mail or fax.
